Most conversion houses run a scanned book through an automated OCR engine, run a second automated pass to catch the obvious errors, and ship it. That is where the mistakes get baked in, because no one ever reads the result against the original. Our OCR process is our own, and it depends on human interaction rather than automation. A person checks every single page against the source.
